| Customer Guarantee: | Limited Warranty: We warrant
products sold to be free from defects in materials and workmanship under
proper use and storage, for a period of 12 months from the date of the
original shipment. Our liability and obligation under this warranty shall be expressly limited to the repair or replacement, as we may determine at our sole discretion, of any defective merchandise for which the buyer has given notice to us. No warranty claim will be allowed which, in our opinion, resulted from misuse, negligence, accident, or repair by an entity other than LifeLines Neurodiagnostic Systems, Inc. Other than the repair or replacement of defective products under the limited warranty set forth above, we shall have no obligation to the buyer with respect to any monetary damages by any defect, and in no event shall we be liable to the buyer for any lost profits or consequential damages. |

| Return Policy: | Items may be returned for refund
or replacement within 30 days of the shipment date. You must obtain a
Return Merchandise Authorization number from us prior to the return of
any items. All returns must be accompanied by the packing slip. You may
want to insure your return; we cannot be responsible for lost or
misdirected returns. For merchandise returned for refund, credits will
be issued to your credit card and will appear on your billing statement. Shipping and handling charges are non-refundable.
